Book your appointment in advance in order to reserve your requested date and time.
Client Arrivals and Parking:
Forms:
In order to maximize your time in our office, we ask that your forms be filled out prior to your appointment. Forms can be downloaded and printed from the Download Patient Form link above.
Parking:
We are located on the second floor, Suite 2300 of the Providence One building. You may park in the paid parking structure adjacent to the Providence One building. Parking is free for the first 30 minutes, then $2.00 every 30 minutes.
Validation:
We do not validate parking, however, King's Pharmacy in the Providence One building will glady validate your parking if you fill your prescriptions there.
If you are a first time patient, please plan to arrive at least 30 minutes prior to your appointment in order to find parking, relax and enjoy our amenities.
Late Appointments:
If you arrive more than 15 minutes late for an appointment, you may be re-scheduled in order to accommodate patients who have appointments following yours.
Rescheduling, Cancellations and "No Shows":
If you need to reschedule a cosmetic procedure, please notify us at least 48 hours in advance so that we may accommodate other patients.
In the event of cancellations or "no shows" less than 48 hours in advance for a cosmetic procedure,a $100.00 no show fee or the cost of the procedure (whichever is less) will be charged to the credit card on file, or deducted from your pre-paid package.
